Before you begin

To help you complete the referral smoothly, please ensure the following information is available. These items are guidance only.

📋 Learner Information

Full name, date of birth, current educational provision, year group, and primary areas of need.

✅ Consent

Confirmation that parental or carer consent has been obtained to share information, and that the pupil is aware of and agrees to the referral where appropriate.

📄 Supporting Documents

Relevant documentation such as EHCPs, professional reports, risk assessments, attendance data, or medical evidence.
